Service Coordinator

MERCY Communities is looking for a professional in the social work field who is passionate about helping the underserved in the Springfield area to join our team. MERCY's mission is to provide housing and supportive services to foster the independence of homeless and at-risk families.

The Service Coordinator (case manager) duties include:

  • Participate in the intake and assessment of client families and orient clients to the program expectations
  • Identify, assess and prioritize immediate and long term needs of participants and utilize that assessment in working with the client head of household to create and implement the participant’s service plan and goals. Continually update assessments upon receiving new participant information
  • Provide referrals and assistance as needed. Assess for benefit entitlements and ensure enrollment
  • Conduct site visits and one-on-one case management meetings on a weekly basis to discuss progress toward goals, identify potential barriers and relevant solutions
  • Coordinates all in-house and off-site services offered to participants
  • Assesses the needs of the participants’ children, i.e. day care, education, counseling, development screenings, health care exams, etc. and refers accordingly
  • Maintain client demographic and case management data in homeless management information software database.
  • Provide rides to clients to medical appointments, and other previously arranged transportation (in MERCY owned vehicle)
  • Develop a discharge plan for participant’s move into permanent housing in the community
  • Other duties as assigned

The ideal candidate will have a minimum of a bachelor's degree, case management experience, a valid Illinois driver's license and vehicle insurance and can pass a background check, CANTS check and drug screening.

37.5 hours per week M-F. Pay Rate: $40,000 per year plus excellent benefits.
